  Your 1095-C tax form for 2023 medical coverage is now available.

  On February 27, 2024, the tax form was mailed to your home address on file. You can also print your tax form by visiting mytaxform.com. Access to this site and/or printing the materials may be restricted in the workplace. To set up an account:

  1. Visit mytaxform.com.
  2. Enter “Maximus” in the Employer Name/Code field and follow the prompts to identify the employer.
  3. Register as a first-time user and enter your personal information to create an account.
  4. Follow the prompts to set up to receive your tax form online.

  Note: If the 2023 1095-C tax form does not appear in your account, email benefits@maximus.com for assistance.

  What is a 1095-C tax form? The 1095-C tax form is issued by employers to show the amount of health insurance coverage that was provided to an employee during the year. It is used to help determine the amount of the premium tax credit that an individual may be eligible for.
